Minnesota — CDCS (Consumer Directed Community Supports)
What is CDCS?

CDCS is a Minnesota DHS waiver option that allows individuals and families to direct their own support services based on ISP goals.

Find Love Safely services may be authorized under CDCS when aligned with goals such as:

  • Reducing social isolation
  • Increasing safe community participation
  • Building communication and relationship skills
  • Supporting decision-making related to boundaries and consent
What services may be authorized?

CDCS does not approve “memberships” as a blanket service. Instead, individual components may be authorized.

Eligible service components may include:

Facilitated Social Connection & Matching Support

Human-led introductions aligned with preferences and safety considerations.

Group Relationship Education (Virtual Workshops)

45-minute virtual sessions (up to two per month) covering:

  • Friendship skills
  • Communication
  • Boundaries and consent
  • Online safety
  • Dating readiness

Individualized Support Check-Ins (Concierge level only)

Quarterly 30-minute administrative check-ins via Zoom to review:

  • Living situation changes
  • Caregiver involvement
  • Preferences and goals

Individualized Relationship Support Sessions (Optional)

60-minute virtual sessions focused on reinforcing social and relationship skills.

Pricing (Private-Pay Reference)

Core Program: $349 per year

Essentials Program: $1,299/year or $129/month (3-month minimum)

Concierge Program: $2,799/year or $259/month (3-month minimum)

Massachusetts — Self-Directed Flexible Funding
Can I use self-directed or flexible funding for Find Love Safely?

Yes. Individuals in Massachusetts who have self-directed funding through programs such as Agency with Choice (AWC) or Participant-Directed Program (PDP) may be able to use those funds for Find Love Safely services.

These programs often include flexible funding stipends, which can be used for goods and services that support goals like social recreation, community inclusion, independence, and relationship-building.

Because these funds are tied to broader life goals (not specific services), Find Love Safely can be included as part of a plan that supports building friendships, social skills, and meaningful relationships.

We recommend speaking with your DDS service coordinator about how relationship-building and social connection goals can be incorporated into your funding plan.

Our team is happy to provide documentation or answer questions to support the process when needed.

Availability and approval may vary based on individual funding status and DDS guidelines. Individuals without existing stipends may inquire with their service coordinator, though funding is need-based and not guaranteed.

What if I don’t currently have flexible funding in Massachusetts?

Individuals who do not currently have flexible funding can speak with their DDS service coordinator about whether they may be eligible for a stipend through AWC or PDP.

Funding is need-based and not guaranteed, but families can request support for goals related to social connection, community participation, and independence.
